Standard – less than 1 year after occurrence date

If the mother is single/in common-law/ divorced or has a visiting partner:

  1. Proof of Birth issued by Medical facility

  2. Mother’s identification document.

  3. Father’s identification document – if father’s details are to be added.

  4. Father’s consent – if father’s details are to be added

If the mother is married or a widow:

  1. Proof of Birth issued by Medical facility.
  2. Mother’s identification document
  3. Father’s identification document
  4. Marriage certificate
  5. Death certificate of Father – if the mother is widow

If mother deceased:

  1. Proof of Birth issued by Medical facility
    Mother’s identification document
  2. Father’s identification document – if father’s details are to be added.
  3. Father’s consent – if father’s details are to be added
  4. Death Certificate of Mother 

Late registration – more than one year after birth

If the mother is single/in common-law/ divorced/married/widow or has a visiting partner:

  1. Proof of Birth issued by Medical facility
  2. Mother’s identification document
  3. Father’s identification document – if father’s details are to be added
  4. Father’s consent – if father’s details are to be added
  5. 2 signed Declaration forms from 2 witnesses
  6. If the birth took place at home, then 4 signed Declaration Forms are needed from the mother, father, and 2 witnesses
  7. In the absense of proof of birth, baptism record and/or school attendant record can be submitted.

You can register your child’s birth through the online Civil Registry and Vital Statistics System (CRVS) or by visiting any of the Vital Statistics Unit offices.

For late birth registration between 1 and 5 years, the fee is BZ$25. For late registration after 5 years, the fee is  BZ$50.

Yes, the Father’s name can be added throught the Birth Amendment Service. The fee for this service is BZ$8.50.

Any person with an original Medical Certificate for Cause of Death issued by the Ministry of Health and Wellness can register the death.

  4. Marriage Registry issued by the Marriage Officer
  5. If authorised person is applying on behalf of groom or bride, then signed authorization form is needed. 

*An ID of the applicant (Passport or Social Security Card. National ID is accepted for foreigners). Driver’s license and Voter’s Card are not accepted.

The Adoption is registered by the Court only. Adoptive parents can apply for the Adoption Certificate through the Information Services.

A court order is required to apply for adoption service.

Children adopted outside Belize are not eligible for this certificate.

A deed poll is a legal instrument used to officially change a person’s name. It is required when an individual wants to change their name for official purposes.

  2. Parents consent form if changed for minor

No, any name change to a deed poll can only be done through an application for a new deed poll.

Yes, the person who submitted the application on CRVS will receive a unique “Collection Authorization Form” with key details about the application. This form is a requirement for collecting a paper certificate. If you would like someone to pick up your certificate, simply provide them with a digital or physical copy of this form. The person picking up the certificate must present a Social Security or Passport identification.

 

Note that a person can only gain access to this form if you share it with them. With that in mind, be mindful of who is given access to this form.

Notifications will be sent once the applicant provides an email or phone number. If none are provided, then VSU cannot provide status notifications.

The Vital Statistics Unit is introducing e-certificates, providing applicants with the flexibility to request either an electronic certificate, a traditional paper certificate, or both formats. The e-certificates can be conveniently printed at home and are acceptable for all purposes. Both the e-certificates and paper certificates will feature a QR code that enables any entity to quickly confirm that the certificate is valid by scanning the code. This innovative approach ensures that both paper and electronic certificates are secure and verifiable.

Yes, all VSU offices are prepared to offer any type of service. You are also able to pick up your certificates from the VSU district offices.
